2. Have you been in close contact with anyone who has tested positive for COVID-19 or is suspected of having COVID-19?

3. In your household, is anyone self-isolating for 14 days as a result of travelling internationally?

You've answered "YES" to one or more of the questions above. HR requires you to complete the Health Canada Self-Assessment Tool or the CDC Coronavirus Self-Checker. Please check the box below to acknowledge that you will do this after submitting this form. A link to these tests will be provided on the next page.

What is this pre-work check-in questionnaire?

Capital Automotive Group uses this pre-work check-in to conduct a daily assessment of all employees who are returning to work. It’s important to do this each and every day an employee works in order to help with contact-tracing and to help avoid the spread of COVID-19.

What should I do if I’m not feeling well?

If you’re at home and you’re not feeling well, please do not come to work. Follow the guidelines outlined by the Government of Canada and your local health authority and stay home. If you’re at work and starting to not feel well, speak to your manager to discuss what you should do.

What if I notice a coworker is showing symptoms of COVID-19?

Contact your store’s HR Manager or Shelley Schiller if you feel a coworker may be showing symptoms or is unwell.
